REM Wisconsin proudly offers the REM Heritage program for adults with Alzheimer’s disease and related dementias. Through REM Heritage, individuals live in our licensed Adult Family Homes and Community-Based Residential Facilities, sometimes called group homes.
We developed the REM Heritage program as part of our commitment to providing individuals of all abilities with the supports they need in the most natural settings possible. Our impeccably maintained homes for individuals with Alzheimer’s disease and related dementias are an alternative to congregate care and nursing facilities.
Our friendly, specially trained staff provide 24/7 in-home support. And with low staff ratios (1:4 or less), we are able to provide individuals with the personal support and attention they need. With REM Heritage, individuals benefit from the freedom of participating as they choose in home and community life while receiving round-the-clock support and a range of personalized services.

Memory Supports
We understand people with Alzheimer’s disease and related dementias have special needs, and we ensure our services and homes are equipped to meet them. To promote long-term memory, we base activities on the individual’s past experience. These may include sewing, woodwork, or playing music or favorite board games. We also develop personalized Memory Books and provide iPads with preferred music, pictures of family and friends, and dementia-related apps.
Like the services we offer, REM Heritage homes are also designed to support individuals with Alzheimer’s disease and related dementias. Because people with dementia often experience vision challenges, our homes included blended transitions between rooms to prevent visual cliffs and brightly painted interiors to create a warm, dynamic environment. Our homes also have fenced backyards with raised gardens and indoor exercise and activity areas.
